The Nugent Commission of Inquiry final report has recommended that an Inspector General be appointed to the South African Revenue Services (SARS).
The report was handed over to President Cyril Ramaphosa on Friday, after he appointed the commission in May to probe administration and governance at the tax agency.
"This role, properly performed, had it been in place during Mr Monyane's incumbency, would in all probability have prevented much of the damage and destruction which the commission has outlined in its interim report," the Nugent report reads.
The interim report in October recommended former SARS commissioner Tom Moyane be removed immediately, saying he had damaged the revenue agency.
The final report has also recommended that a deputy SARS commissioner be appointed "to provide continuity in the event that the commissioner is unable due to incapacitation or any other reason to perform the functions of the commissioner".
The report further recommended a deputy commissioner be appointed to safeguard "against the unfettered exercise of power by the commissioner".
A presidency statement said Ramaphosa is "studying the report and will apply his mind to its detailed recommendations".
The president thanked retired Judge Robert Nugent, professor Michael Katz, adv. Mabongi Masilo and Vuyo Kahla "for the diligence and commitment with which they undertook this important task".
